I've made 13 clinical supply mistakes so you don't have to: why checking beats fixing

Confidence is the first thing I got wrong

It's a solid standard. But here's the catch: it guarantees what the manufacturer does. It can't guarantee what happens after the box hits your loading dock. Seals can fail in transit. Moisture can get in. A pouch can be punctured by a poorly packed neighbouring box. Unless somebody actually looks, the standard means nothing.

The ventilator near-miss that rewrote my checklist

Then there was September 2022. This one still makes me cringe.

A mechanical ventilator came back from scheduled service. The biomedical team had signed off, the service sticker was fresh, and the unit sat in the equipment bay ready for assignment. I approved it for ICU use based on that sticker alone. Didn't touch the device myself. Why would I? It had just been serviced.

That's the assumption that almost cost someone their airway.

The next morning, a nurse did her pre-connection check—a step I had previously considered "belt and braces." She found the alarm limits had been reset to factory defaults during servicing. The high-pressure alarm was set at a level that would have delayed an obstruction alert until pressures were dangerously high. Not a ventilator malfunction. A configuration mismatch. The ventilator was perfectly happy to run with the wrong settings; only a human check caught it.

Even after hearing her feedback, I kept second-guessing myself. What if she hadn't checked? The two hours between her finding and the official incident report were stressful, I'll admit. I'd hit "approve" and walked away, and it took a senior nurse to catch what my verification step should have caught.

Here's the anti-intuitive part: a recent service sticker isn't a sign that no check is needed. It's a sign that a check by someone else happened—at a moment when what you need is a check for your use case. The biomedical team verified the ventilator works. They didn't verify the alarm limits match our ICU's clinical protocol. Those are different questions, and ECRI Institute's annual health technology hazard list has flagged misconfigured ventilator alarms as a top risk for years. I'd read those warnings. I still walked past the exact scenario.

Reading vital signs is a trend skill, not a snapshot skill

The third lesson isn't about equipment or packaging at all. It's about the data the equipment gives you—and how badly we teach people to read it.

I've lost count of the times I've seen a clinician panic over a single cardiac monitor reading, or call a patient "stable" based on one set of vitals that happened to fall in range. I've done both. The second one was worse.

In my second year, I documented a patient as stable during an observation shift. Heart rate 82, blood pressure 128/74, SpO₂ 97%. All fine. But I hadn't looked at the previous twelve hours of the cardiac monitor's trend. The trajectory showed a slow rise in heart rate, a slow drop in blood pressure, and declining SpO₂—each individually "normal," together a textbook picture of early deterioration.

The nurse who took over spotted it within minutes. The patient was transferred to ICU the same hour. That was the closest I've come to a serious harm event, and it taught me more than any course ever did.

So when someone asks me "how to read vital signs," my answer is always the same: don't read the number; read the curve. The monitor isn't a hand of cards; it's a sentence. Each reading is one word, and you can't understand a sentence by staring at a single word.

It's at least forty. Three would have caused serious patient harm. Those three alone justify the whole thing.

Do the math. Five minutes of verification per checklist versus incident reports, root-cause analyses, and the cost of a preventable adverse event. I estimate the checklist has saved us around $8,000 in averted rework and product loss. The patient-safety dividend is harder to quantify, but I know this: those three catches would have been my fourth, fifth, and sixth sign-off failures. I'm not willing to carry that.

The five-minute rule

So here's my position, and I'm not softening it for anyone: the most expensive thing in clinical supply is the five minutes you skip because you're confident.

I don't expect every clinician to distrust their equipment or their supplies. I expect them to verify what their own role is responsible for. The manufacturer handles sterility at their end. The biomedical team handles function at their end. Your job is to confirm that the product you're about to use is what you need, in the condition you need, configured the way your patient needs. That's not redundancy. That's the system working as designed.
